2023-2024 Student Health Insurance Information

Health insurance coverage is required of all undergraduate resident and non-resident students who are enrolled full time at the college. Graduate students can request enrollment in the student insurance plan by contacting the Student Accounts Office for details.

All Full-Time Undergraduate Students are automatically enrolled in and billed for the Student Health Insurance Plan (SHIP). If you are insured by a plan that provides comparable coverage, you can request to waive the 2023-2024 SHIP.

Academic HealthPlans (AHP), is the Plan Administrator for Lake Forest College’s UnitedHealthcare (UHC) Plan for the 2023-24 plan year.

You will be automatically billed for the UHC Student Health Insurance Plan (SHIP) at registration and will be required to OPT-IN to the plan unless proof of comparable coverage is provided by completing an insurance waiver. If you plan to waive, your current insurance must be fully compliant with the Affordable Care Act and provide access to doctors near your school. Please note, you should have your health insurance ID Card available. If you would like to review the policy coverage prior to completing one of the processes below, please go to lakeforest.mycare26.com.

ACTIVATE COVERAGE/OPT- IN

To accept enrollment in the UHC SHIP, follow these steps after logging in at students.care26.com:

  1. Click the "I want to Enroll" button on your dashboard.
  2. Select your plan to enroll in SHIP, Add to Cart and click "Proceed to Checkout".
  3. Select the "one-time payment" option
  4. Select your payment type"Charge through School" - NOTE: You will not be required to pay at this time. The charge for the health insurance has already been billed to your student account.
  5. You are now enrolled in the insurance and will receive confirmations to your school email address.

WAIVE OUT of COVERAGE

If you have private health insurance that provides coverage in the State of Illinois and meets the waiver requirements, follow the instructions below to submit a waiver before the Fall semester deadline date of 08/15/2023.

Have an electronic copy of your current insurance card and your full policy document ready.

  1. Go to students.care26.com and enter your @lakeforest.edu email.
  2. Click "Log in using my Lake Forest Account".
  3. After logging in to your Lake Forest College account, you will be taken to the Care26 dashboard.
  4. Click on "I want to Waive" and confirm you meet the waiver criteria.
  5. Enter your insurance information and attach your required insurance documents (an electronic copy of the front and back of your current insurance ID card and full policy document), then submit your waiver. 

Please note: Academic Health Plans will audit or verify your waiver request. This is to make sure your insurance plan will cover you when you’re at Lake Forest College. Here’s how the waiver review process works:

  • Academic Health Plans will check the insurance company information you entered on your form to make sure it’s accurate and that your coverage is active.
  • Academic Health Plans will verify most waiver requests within 1-7 business days.
  • Once Academic Health Plans verifies your coverage, they will email you at the address you provided on your form to let you know if your waiver request was approved or denied.

Dental and Vision are additional products available for purchase separately even if you do not purchase the Medical Student Health Plan. Even if you waive, Voluntary Dental Insurance and Voluntary Vison Insurance is available to all students on the Lake Forest College Mycare website (mycare26.com) under Additional Products Available.

Cigna Dental - Students and dependents who wish to enroll in dental coverage will need to complete the online enrollment process and pay the full premium amount online via credit card to Cigna.

VSP Vison - Students and dependents who wish to enroll in vison coverage will need to complete the online enrollment process and pay the full premium amount online via credit card to VSP. REMEMBER: You will be automatically enrolled in the Non-Refundable SHIP if a waiver form is not successfully submitted by the deadline of August 15, 2023 for Fall and by January 15, 2024 for Spring/Summer only coverage.

Students Studying Abroad

Students studying abroad are required to have medical evacuation and repatriation coverage. This is covered by the College student health insurance plan.
